Medication History Consent

As of April 15th, new patients will receive a new consent form titled “(TF) – Medication History Consent” Figure 1. While we do get consent during the patient screener for checking PDMP, this is a general medication history consent to give permission for us to also access unscheduled medications via the SureScripts database.

Consent to Obtain Patient Medication History

Patient medication history is a list of prescription medicines that are practice providers, or other providers, have prescribed for you. Variety of sources, including pharmacies and health insurers, contribute to the collection of this history.

The collected information is stored in the practice electronic medical record system and becomes part of your personal medical record. Medication history is very important in helping healthcare providers treat your symptoms and/or illness properly and in avoiding potentially dangerous drug interactions.

It is very important that you and your provider discuss all your medications to ensure that your Record his medication history is 100% accurate. Some pharmacies do not make drug history information available, and your drug history might not include drugs purchased without using your health insurance. Also, over-the-counter drugs, supplements or herbal remedies may not be included in your record.

I give my permission to allow my healthcare provider to obtain my medication history from my pharmacy, my health plans, and my other healthcare providers.

Patient Demographics

When a patient gives consent for our practice to review their medication history, this should be noted under Patient Details > Demographics > Additional Information as illustrated below in Fred Testerson’s chart under the field “Rx Hx Consent”.

 

Questionnaires

Just as with all other submitted questionnaires, the medication history consent questionnaire will be visible from the patient details section under the questionnaires column.

If a patient does not have a Medication History Consent form on file, you can send them the “(TF) – Medication History Consent” questionnaire manually in Charm.

You can also ask the patient verbally and then update their demographic section as detailed above. Even if you get verbal consent, please also send the questionnaire via Charm.

The short answer is yes. Earlier versions of the screener (Prior to April 21, 2022) asked for consent to prescription history via OK PDMP. It has since been updated to include SureScripts.

The other reasons the consent form was added is that not all patients complete the screener form, and or the patient may have never been asked about medication history in the past.
